2014 Evidence Cover Form-Required form for all student work samples submitted.

2014 Observation Cover Form-Required form to document teacher observations of the student’s performance.

2014 Photograph Series Cover Form-Required form for a series of photographs submitted as evidence.

2014 Video Comment Form-Complete this form prior to submitting a video as evidence to have all the information needed for the AVS submission.

MAAESF Ethics Form*- Required form for each student's portfolio.

MAAESF Media Permission Form -Required form if any pictures or video showing a student’s face is submitted as evidence. Would be submitted for each student in a media file. Requires parent’s signature.

MAAESF Non Recognizable Student Media Form -Required form if any pictures or video showing any students, even if they are not recognizable. This would be submitted for each student in a media file.
