Evidence Cover Forms
2012 Evidence Cover Form
Required form for all student work samples submitted
2012 Observation Cover Form
Required form to document teacher observations of the student’s performance
2012 Photograph Series Cover Form
Required form for a series of photographs submitted as evidence
2012 Video Comment Form
Complete this form prior to submitting a video as evidence to have all the information needed for the AVS submission

MAAECF Ethics Form*
Required form for each student’s portfolio.
MAAECF Homebound Student Form*
Required form for any student who will be assessed through homebound instruction.
MAAECF Media Permission Form
Required form if any pictures or video showing a student’s face is submitted as evidence. Would be submitted for each student in a media file. Requires parent’s signature.
MAAECF Non Recognizable Student Media Form
Required form in any pictures or video showing any students, even if they are not recognizable. Would be submitted for each student in a media file.
MAAECF Progress Determination Form*
Required form for any student being assessed under the Progress Rubric.
MAAECF Repeat Grade Form*
Required form if a student will be completing the MAAECF on a grade level in which they’ve been previously assessed.
MAAECF Repeat Objective Request Form*
Required form to request permission to repeat an objective on which a student has been previously assessed.
MAAECF Student Transfer Form*
Required form if a student transfers in to a district from either another state or another district in Mississippi.
MAAECF Correction Form*
Required form to correct information found to be incorrect in the AVS system. AVS will open mid-December. Student Correction Forms will not be accepted until after AVS has opened and the district has checked the student data file in AVS.
