Contracts

24.  Mississippi State Board of Health.  $42,700 (subject to availability of funds).  03/21/05 – 06/30/05.  Sole source.  Federal funds 

The purpose of this contract is to provide required inspections of sites for participation in the Summer Food Service Program.  The Mississippi State Board of Health is the governing body, charged by State law, with the responsibility to regulate food service sanitation.  Therefore, this contractor is the sole source for procurement in Mississippi for this purpose.  All food served in foodservice facilities in this state are subject to regulations and sanitation inspections by Mississippi State Board of Health.  The contractor is located in Jackson, Mississippi.  The contractor is not a former employee or related to any Department employee.

America’s Choice, Inc.  No MDE funds involved.  06/30/05 – 06/30/07.  Request for Proposals.  Multi-year contract

The contractor will implement and facilitate a system of services to support K-12 schools that do not meet Adequate Yearly Progress (AYP).  AYP refers to the formula specified in No Child Left Behind for determining whether schools and school districts have met adequate yearly progress criteria. It holds all schools and districts to a fixed set of annual objectives based primarily on the results of statewide assessments. The contractor will provide training, professional development, technical assistance and materials for school personnel in the areas of leadership, literacy, mathematics, curriculum alignment assessment and community involvement related to whole school reform.  School districts may purchase these services by entering into a contract with America’s Choice, Inc. based on the established fee structure, using available federal, district, or local funds. The contractor is located in Washington, DC.  Department staff was unable to reduce the cost to districts by negotiation.  The personnel associated with this contract are not former Department employees or related to any Department employee.                         

03.       Approval of contracts for State Public School Building Fund            

The Office of Safe and Orderly Schools requests approval of contracts for state public school building funds.  School districts are requested to submit an agreement that has been made and properly executed between the school district and contractor for capital improvements. 

04.       Approval of change orders for State Public School Building Fund

The Office of Safe and Orderly Schools requests approval of change orders for construction projects.  School districts are requested to submit any modifications to an approved contract. 

05.       Approval of final projects for State Public School Building Fund

The Office of Safe and Orderly Schools requests approval of final projects for state public school building funds.  School districts are requested to submit closing documents upon completion of construction projects. 

06.       Approval of project cancellation for State Public School Building Fund

The Office of Safe and Orderly Schools requests approval of project cancellation.  Construction projects may be canceled at the request of the school district.

07.       Approval of school district facility site(s) for the location of the construction of capital improvements

DeSoto County School District requests approval of a site for the construction of a new high school (grades 9 through 12 with an enrollment of approximately 1,750 students) located on approximately 33 acres on Rasco Road in Southaven, Mississippi.
